POSITION OVERVIEW AND RESPONSIBILITIES:

This is a highly responsible position under the direction of the statewide Transaction coordinator. This position assists with centralizing the administration of Long-Term Care Ombudsman Program (LTCOP) processes and responses regarding facility-initiated transfer/discharge of nursing home, assisted living facility (ALF) and Adult Family Care Homes (AFCH) residents, and providing comprehensive oversight and assistance. This position also provides technical assistance and guidance for LTCOP to ensure statewide coordination and efficiency regarding the transition of residents due to transfers/discharges.  This position coordinates with other DOEA programs (CARES, OPPG, etc.), and external agencies (AHCA, DCF, Law Enforcement) as required.

PAY:

Salary will be commensurate with experience and qualifications, ranging from $35,317.32 to $39,520.08, annually.

DUTIES AND RESPONSIBILITIES:

  • Coordinates with Lead Transitions Advocate and monitor reports from Adult Protective Services (APS) received at the district level to identify trends in complaints.  Work with District Ombudsman Managers to ensure proper tracking of all APS reports (substantiated and unsubstantiated). Coordinate follow-up response to problematic facilities based on APS reports received.  Coordinate follow-up response to problematic facilities based on APS reports received.
  • Works closely with Regional and District Ombudsman Managers on cases and complaints as reported through APS involving transfer/discharge and abuse, neglect and exploitation of seniors.
  • Assists with LTCOP response related to involuntary transfer/discharge of consumers from long-term care facilities statewide;
  • Assists as receiver of transfer/discharge notices.  Reviews notices to determine which are appropriate versus inappropriate actions, and if the notices comply with federal and state standards (C.F.R. § 483.15, etc.). Disperses notices requiring action to the appropriate region/district.  Provides guidance and coordinates with regions/districts regarding the transition process.
  • Assist Lead Transitions Advocate and coordinate with Regional and District Ombudsman managers to ensure data is tracked, entered, and maintained on a regular basis.  Monitors data-collection system (transfer database) for facility-initiated transfer/discharge notices, to ensure consistency among facilities throughout the state.  Provides guidance for analysis and reporting of data from these notices; includes system for statewide tracking of related cases and appeal hearings.  Data to include notice compliance with C.F.R. § 483.15 standards, contacts made, actions taken, visits made, resulting actions, etc. Data will be housed in the Collabrios system (LTCOP database). Coordinates with Operations Analyst II to ensure tracking data is entered and maintained on a regular basis.
  • Assists with the review of transfer/discharge complaints to determine systemic issues that may vary throughout the state and identify regional, statewide and/or national trends in care.  Collects data and conducts research to plan and direct system-wide advocacy efforts. 
  • Diseminates a standard notice template to help nursing homes provide complete and accurate information to residents facing discharge and to LTCOP and comply with C.F.R. § 483.15 standards.  The template should include instructions and definitions that help nursing homes provide the information that CMS requires to be in the notice and information about LTCOP and other resources available. 
  • Follows processes and policies for managing receipt and monitoring of nursing home discharge notices including actions to take if notices do not comply with the federal and state requirements.  Ensures documentation is properly shared with Centers for Medicare & Medicaid Services (CMS) when requested. 
  • Provides data for the monthly Involuntary Discharge Reports to the State Ombudsman, Legal Advocate, and Deputy State Ombudsman (Field Operations).
  • Based on research findings, assists legal advocate with drafting legislative policy needed to improve resident transfer, discharge and relocation rights.
  • Develops and maintains relationships with legal aid providers, hospital systems, homeless shelters, insurance case managers to address inappropriate transfer/discharge activities. Develop tracking systems to record all contact information.
  • Collaborates with Resident Council presidents to identify facility training opportunities; track all community outreach data in Collabrios system (or other database/tracking system);
  • Monitors facility closures and ensure consumers are promptly provided with information regarding available long-term care services to enable consumers to select new providers; track resident transition to the new facility.  Coordinate with regional/district staff to facilitate transition. May conduct or participate in on-site reviews of nursing home facilities.
  • Works with Transitions Advocate Coordinator to develop training resources for transfer/discharge-related training/CEUs for field ombudsman in collaboration with Training Coordinator.
  • Provides training/updates, information for Nursing Home/ALF/AFCH staff (due to staff turnover) on the discharge/transfer process, correctly completing the Standard Notice Template and process for notifying LTCOP (where to send notifications, etc.)
  • Serves as primary office contact in the absence of the Transition Advocate;
  • Performs other duties as required;
  • Travel/field work and driving as required.  May require overnight travel.
  • Attendance is an essential function of the position.

 

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Ability to plan, organize, and ensure work is completed correctly, timely, and in accordance with applicable laws, regulations, rules, policies, and directives. 
  • Ability to read, understand, and apply federal, state, and local laws, regulations, policies and procedures governing LTCOP. 
  • Ability to fulfill all ombudsman program certification and continuing education training requirements.
  • Ability to establish and maintain effective working relationships with others.
  • Ability to communicate effectively, whether in person, or by telephone or email, in a courteous and effective manner.
  • Ability to conduct thorough investigations, assessments and fact-finding interviews.
  • Ability to obtain and verify information; interpret written, numerical, and verbal data; analyze evidence, and resolve complaints
  • Knowledge of basic.
  • Knowledge of investigative techniques including proper research methods for gathering data and ability to present research in a concise, coherent and logical format resulting in accurate investigative reports.
  • Ability to perform basic arithmetic calculations.
  • Ability to make independent decisions.

  • Proficiency in Microsoft Office Suite and Windows 2000, general office equipment;  ability to learn new programs; and ability to work with database systems;

REQUIREMENTS:

  • If selected for employment applicant will be required to become a certified Ombudsman upon time frame set by employer.
  • Travel is required.  Operates own private motor vehicle for routine travel within the assigned office's planning and service area. All applicable State and local driving laws and insurance regulations must be strictly adhered to in the operation of the private vehicle while performing State business. You must possess a valid Florida driver’s license.
