POSITION OVERVIEW AND RESPONSIBILITIES:

This role calls for a highly responsible and effective individual to assist the Alzheimer’s Disease and Related Dementia (ADRD) population in obtaining the necessary services to ascertain optimal medical care and support services for home-based care. The person in this role is responsible for creating and maintaining trusted relationships with caregiving families for an extended period. Consultations performed by the individual in this role will become ongoing case management-style clients. The individual in this role must provide excellent follow-through, record-keeping, and problem solving. This individual must remain up to date with knowledge of resources and emerging ADRD research findings. The person in this role will serve a specific territory as outlined in the Planning and Service Area map (as assigned) and should have high-level knowledge of the resources available to families in the region they serve.

 

Additionally, as the Lead Care Navigator, this individual will oversee and support a team of Care Navigators, ensuring consistent and high-quality service delivery across all assigned territories. The Lead Care Navigator will mentor, train, and provide guidance to team members while also acting as a liaison between the Care Navigator team and senior leadership.

 

This position reports to the Florida Alzheimer’s Center of Excellence manager and Dementia Director. The individual will be based out of Department Headquarters in Tallahassee and may work remotely within the assigned territory. The Lead Care Navigator will engage regularly with the Department’s Dementia Director, the Professional Review Team, and provide supervisory support to the Care Navigator team.

 

PAY:

Salary will be commensurate with experience and qualifications, ranging from $44,060.16 to $55,000.08, annually.

DUTIES AND RESPONSIBILITIES:

Direct Care Navigation Responsibilities:

  • Provide education, guidance, and support to families on ADRD issues by phone and/or email, as needed, including timely:
    • Development of individualized care plans
    • Follow-up with families, when indicated
    • Documentation of all interactions with PLWD and caregivers
    • Practice empathetic listening and validation techniques
    • Maintain confidentiality, as required
  • Support the broader services offered through the Department that impact the overall family caregiving effort (e.g., Elder Helpline, services available through the Alzheimer’s Disease Initiative, Memory Disorder Clinics, etc.)
  • Participate in pilot/special projects, which involve collaborative efforts to improve assessment and delivery of services

 

Leadership and Team Management:

  • Provide supervision, mentorship, and guidance to the Care Navigator team, ensuring consistency and quality in service delivery
  • Lead regular team meetings to discuss case management strategies, challenges, and best practices
  • Train new Care Navigators and ensure they have the resources and support needed to perform their roles effectively
  • Monitor team performance and provide feedback to enhance individual and overall team effectiveness
  • Act as the primary point of contact for escalated cases that require additional expertise or intervention

 

Build Relationships and Strengthen the ADRD Network:

  • Develop ongoing relationships with and provide education for key personnel
  • Collaborate with the Dementia Care and Cure Initiative Task Forces to enhance the network of local resources and collaborative efforts with other entities (e.g., ACHA, DOH, DCF, Florida’s MDCs, Brain Bank, Alzheimer’s Association, AARP, and other external partners)

 

Remain Knowledgeable:

  • Remain knowledgeable and current in information on ADRD, as well as national, state, and local programs and services that affect the targeted population
  • Stay abreast of rules and regulations guiding ADRD care in facilities
  • Improve/enhance professional knowledge, skills, and abilities through education courses, seminars, workshops, review of policies and procedures, etc.

 

Inform Future Work:

  • Proactively anticipate gaps in ADRD services and programs to resolve issues through a collaborative decision-making process with key persons within the Department or external stakeholders to address short or long-term needs or gaps in programs and/or services
  • Available for relationship building and education for primary partners as needed/requested
  • Serve as a consultant to DOEA team members for ADRD resources within assigned regions

 

Meet Goals:

  • Throughout all tasks, meet measurable outcomes through metrics, timelines, benchmarks, etc., to assist with analyzing the effectiveness of specific goals
  • Ensure team-wide adherence to best practices and policies, tracking individual and collective progress toward performance benchmarks

This position combines direct client support with leadership responsibilities to ensure high-quality service delivery while fostering a knowledgeable and collaborative Care Navigator team.

 

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Possess strong written and oral communication skills, required for public speaking and report writing, uses correct grammar, punctuation, and spelling. Is detail oriented and able to enter data accurately into a computerized system.
  • Able to navigate multiple computer screens, applications, and online research engines.
  • Basic proficiency in Microsoft Windows Operating environments and Office Suite products to include Word, Excel, Outlook, PowerPoint, OneDrive, and SharePoint.
  • Ability to prepare clear, concise, and accurate records, reports, correspondence, and other documentation in a logical format within established timeframes.
  • Ability to demonstrate a professional and courteous demeanor.  Ability to establish and maintain cooperative working relationships with the public and staff.  Ability to work independently and with teams at the unit and agency where appropriate.
  • Willing and able to make decisions and exercise sound judgment; to accept responsibility for one’s own actions and decisions; to adapt to changes in work assignments, procedures, and technology; and to be committed to improving individual performance.
  • Willing and able to follow written and oral directions and instructions from management and carry out the directives of persons with duly delegated authority.
  • Willing and able to resolve any differences with management in a constructive manner and accept and abide by management’s direction and decisions.
  • Possess strong project management abilities.
  • Ability to travel statewide.  Ability to lift and carry a laptop and associated documents and equipment for training (up to fifteen pounds).

 

REQUIREMENTS:

  • Florida Driver’s License
  • Public speaking and multi-tasking.
  • Computer usage of Word, Excel and PowerPoint programs. 
  • The ability to multi-task, prioritize work, supervise staff, provide strong leadership skills, and network effectively.
  • Travel and special projects may impact working hours to include evenings and weekends.
