Your Specific Responsibilities:
This position works in a team environment in Bureau of Community Health Assessment in the Health Improvement Planning Section. The professional work performed supports and provides technical assistance to Department of Health divisions, offices and County Health Departments with a focus on community-driven health improvement planning and evidence-informed planning and performance management. These duties and responsibilities support operations, contribute towards improved health outcomes, and fulfillment of Department of Health mission critical activities. Specific responsibilities include the following:
State Health Improvement Planning Process: The team member serves as a content expert and technical advisor on the state health improvement planning process, including the State Health Assessment. They provide technical assistance to the State Health Office and facilitate strategic planning with Priority Area Workgroups towards the creation and maintenance of the State Health Improvement Plan (SHIP).
Community Health Planning and Implementation: The team member acts as a liaison and provides technical assistance to county health departments and community agencies on the community health planning, implementation, and improvement process. They coordinate meeting schedules, conference calls, and travel logistics required to implement community health improvement activities and also coordinate inventory and distribution of related reference materials.
Training and Development: The team member participates in and contributes to the design, development, implementation, and evaluation of training materials related to community health assessment and health improvement planning. They develop unit publications including the SHIP and the Agency Strategic Plan.
Performance Monitoring and Data Management: The team member collects and monitors performance indicators across SHIP Priority Area Workgroups, Department Divisions, and County Health Departments for the performance management system. They enter and interpret data related to performance monitoring measurement and model/promising practices, utilizing a standardized mechanism for collection and dissemination of this information.
Web-Based Resources and Technical Skills: The team member provides expertise and applies technical skills to the production and maintenance of web-based resources for community health assessment and improvement planning, online training, and surveying.
Performs other duties as required, including contract management. Responsible for maintaining the security of information and protecting confidentiality consistent with state and federal laws and rules as well as departmental policies, protocols, and procedures.

Qualifications:
A bachelor’s degree or higher from an accredited college or university in fields such as public health, health administration, social work, or a related field is preferred. Professional experience in any of the following fields: social services, public health, health improvement planning, performance management, strategic planning, program or policy planning and development, program monitoring or evaluation is preferred. The incumbent must also have experience in applying research and evaluation methods, such as collecting, organizing, and analyzing quantitative and qualitative data, identifying and developing findings and areas for improvement, and developing recommendations and approaches to improve performance.
