Your Specific Responsibilities:
This is a highly responsible position that performs a wide variety of duties to collaborate in implementing DOH Leon ’s
regional Breast and Cervical Cancer Early Detection Program and serving as a community health liaison.
Patient Navigation
Provide individualized assistance to clients to help overcome healthcare system barriers and facilitate timely access
to quality screening and diagnostics as well as initiation of treatment for women who are diagnosed with cancer. This
includes but is not limited to: Conduct patient and provider call, send referrals to the appropriate facilities, completion
of the patient report form (PRF), tracking enrollment for screening, re-screening, diagnostic, and treatment services.
Community Engagement Liaison
Plan and execute targeted outreach strategies to increase awareness and enrollment in priority public health
programs (e.g., ADAP, chronic disease screening, immunizations). Coordinate and staff community events, health
fairs, and mobile outreach; tailor messaging for diverse populations and priority zip codes. Provide health education,
resource navigation, and warm handoffs to program staff for eligibility, enrollment, and follow-up.
Partnership Development & Liaison
Establish, maintain, and grow partnerships with CBOs, clinics, pharmacies, provider networks, and social service
agencies. Serve as the point of contact for partners, schedule regular check-ins, present program updates, and
troubleshoot referral barriers. Draft MOUs or partner work plans (as applicable) and monitor deliverables and
timelines.
Reporting
Reports data on the number of women screened, the types of screenings provided, and the demographics of the
screened population. It also tracks program outcomes and utilizes these data for program evaluation and feedback to
providers. Pending Reports are to be completed quarterly. This report involves updating State Health Office on any
outstanding claims, procedures, patient updates, etc. Complete the monthly data report to give a synopsis of the
budget expended, evidence-based programs elicited, how many women were scheduled and screened, events,
providers trained and recruited, etc.
Performs other duties as assigned.
Required Knowledge, Skills, and Abilities:
This position requires experience working in Microsoft Office, especially Excel, high level communication and
interpersonal skills, ability to train and motivate, experience in client coordination and program management.
Adjusting actions in relation to others' actions. Knowledge of privacy laws to protect confidentiality, including
Health Insurance Portability and Accountability Act and applicable state and local privacy laws.
