Job Title: Human Services Program Consultant II

Your Specific Responsibilities:

This position will serve as the lead Disease Intervention Specialist with the regional hub, primary coverage of Gadsden County.

 

Field Investigation and Linkage

As the Lead Field Investigator, the incumbent provides advanced epidemiological and investigative support for Gadsden County and helps, as needed, with investigations that involve both Gadsden and Leon counties. While the position is assigned primarily to Gadsden, the incumbent collaborates with Leon County field investigators when cases include cross-county elements, such as when an investigation originates in one county but involves contacts, residences, or other relevant epidemiological details in the other. This role is responsible for conducting comprehensive communicable disease investigations, including interviewing individuals infected with or at risk for communicable diseases to obtain partner, cluster, and suspect information critical to public health surveillance. The incumbent carries out field investigations to locate clients, deliver Field Directed Therapy, and facilitate access to counseling, medical evaluation, and treatment services. This position ensures that all newly diagnosed HIV-positive individuals in Gadsden County are followed through complete linkage to care, with all activities documented in the HIV/AIDS Linkage modules.

 

The incumbent performs ongoing case management duties, including case documentation, work prioritization, clustering, visual case analysis, and the application of epidemiological techniques consistent with program standards. The role includes providing Rapid HIV and Syphilis testing, delivering test results within required timeframes, and conducting confidential pre- and post-test counseling. The incumbent collaborates with physicians, clinics, laboratories, and other healthcare providers to verify diagnoses, treatments, and laboratory findings, and conducts provider outreach to encourage reporting, share disease trends, and promote adherence to current diagnostic and treatment guidelines. All investigative reports, case documentation, and laboratory-related forms are completed in accordance with CHD and Department of Health policies and procedures.

 

Field Investigation Trainer

In the capacity of Field Investigation Trainer, the incumbent provides instruction, mentoring, and technical support to new Disease Intervention Specialists (DIS) and to DIS staff requiring additional guidance throughout the 14-county service area. The incumbent is responsible for building staff competency in interviewing and engaging with clients in the field, field investigation procedures, linkage-to-care processes, and accurate data collection. Field assessments are conducted quarterly or more often as needed to evaluate staff performance, training needs, and adherence to program standards. Findings and observations from these assessments are submitted to the supervisor and incorporated into the quarterly report provided to county health department leadership. Through ongoing coaching and structured training, the incumbent helps ensure consistent, high-quality field investigation practices across all participating counties.

 

500/501 Trainer As a certified 500/501 Trainer, the incumbent completes the required training and serves as an instructional resource for new DIS staff and community partners who require 500/501 course content. As needed, the incumbent delivers formal training sessions, ensuring all material reflects current program guidelines, legal requirements, and best practices for communicable disease intervention. This role also assists in preparing and presenting the annual updates 500/501 as needed. The incumbent ensures training delivery is consistent, accurate, and supportive of statewide DIS competency standards and collaborates with partners to strengthen community capacity in communicable disease prevention and response.

 

In-Service and Staff Development The incumbent actively participates in in-service training programs, staff development activities, and other professional learning opportunities relevant to program performance. All required Department of Health trainings are completed within established timeframes, and the incumbent maintains current knowledge of emerging communicable disease trends, program protocols, investigative methods, and policy updates. Additional responsibilities may be assigned as needed to support overall program operations and workforce readiness.

STARS reporting system, Linkage to Care module, Leon County HMS system. knowledge of medical and non-medical case management, locally utilized electronic health record systems, interviewing skills, data entry, scheduling of appointments, case coordination with outside agencies/providers. MS Office Suite, MS Teams and virtual meeting platforms.
