The Florida Department of Health (DOH) is responsible for the health and safety of all citizens and visitors to the state. As a public health agency, the Florida Department of Health must monitor the health status of Floridians, diagnose, and investigate health problems, and mobilize local communities to address health-related issues. The Florida Department of Health develops policies and plans that support health goals, enforce laws and regulations that protect the health of all residents and visitors, and link people to needed health care services. The Florida Department of Health also provides specialized assistance to pregnant women, infants, and children with special health care needs, regulates health care practitioners, and provides medical disability determinations.

 

Human Services Program Analyst (Surveillance Coordinator):

This position performs public health surveillance in the control and prevention of Tuberculosis (TB). It calls for a mature and responsible individual able to plan and conduct a program to improve health outcomes and / or key program indicators. The surveillance Coordinator works independently and reports to the TB program Manager. The primary duties are to receive coordinate, monitor, and report public health surveillance activities. The Surveillance Coordinator follows established Federal, State and local laws and Department of Health (DOH) Policies and Procedures.

 

Your Specific Responsibilities:

The Surveillance Coordinator is responsible to receive, track and report on the status of all referrals received by the TB Center.

Referrals arrive from community providers, laboratories, hospitals, concerned citizens, statewide and national reporting systems (e.g., Interjurisdictional Notifications, Electronic Disease Notification (EDN) vie CDC Secure Access Management Services (SAMS)).

Develops on-going communications with health care providers Coordinates with hospitals and other health care providers.

ensuring appropriate and timely referrals of persons suspected or diagnosed with TB in the county. Reviews each referral for reporting delays and makes recommendations for improvement.

  • Ensures the appropriate routing of referrals to nursing and field supervisors.
  • Coordinates with provider and nursing supervisor to monitor classification of persons under evaluation or suspicion for TB within 90 days of referral.
  • Ensures accurate and timely completion of the Report of a Verified Case of TB (RVCT), for each person identified with TB through medical record reviews and coordination with nursing and provider staff.
  • Manages federal and state electronic data systems (e.g., Health Management System, EDN).
  • Accountable and committed to continuous quality improvement and delivering results on program indicators through reporting, analyzing, and taking corrective action as needed.

             Produces surveillance reports as requested; provides monthly data to staff and produces reports on the Epidemiology and Genotyping of TB in Hillsborough County.

             Enters activities into secure database systems for surveillance activities, time accountability, and service collection.

 

Teamwork and Community Engagement:

  • Develops on-going communication with health care providers and surveillance staff in other DOH programs.
  • Coordinates activities with case management team.
  • Maintains honest and open dialogue with staff on patients and surveillance issues.
  • Participates in screening programs, contact investigations, and outbreak or cluster investigations.
  • Works with public and private health related agencies, community agencies, institutions, and individuals in association with services related to TB prevention and control, and consistent with programmatic objectives.
  • Identifies stakeholders, community partners and customers supporting the 10 essential public health services.
  • Attends meetings and events to educate, network, and update the community on TB issues.
  • Provides support and backup to Disease Intervention Specialists and conducts field activities as needed.

 

Training and Development:

  • Participates in grand rounds, in-service trainings, and other developmental and networking opportunities.
  • The Surveillance Coordinator provides information for Department of Health personnel and other agencies in epidemiological. investigation of TB suspect, cases, and contacts, as well as provides community information on TB education.
  • Assists in training and orienting others to the TB Center.
  • Travels, attends meetings, and performs other related duties as needed. The Surveillance Coordinator will use personal vehicle for travel.
  • Assist with Direct Observed Therapy, Video Direct Observed Therapy, and field activity forms as needed.
  • All other duties as directed.

 

 

Required Knowledge, Skills, and Abilities:

Knowledge, skills and abilities, including utilization of equipment, required for the position:

Ability to establish and maintain effective working relationships with others.

Ability to plan, organize and coordinate work activities. 

Ability to operate commonly used office equipment such as computers, printers, fax machine and paper shredder.

Ability to use Microsoft 365 applications (Outlook, Excel, Word, PowerPoint, MS Teams).

Ability to engage with- and analyze new material or information to understand its potential implications.

Ability to effectively communicate with others verbally and in writing, with the ability to actively listen and pose clarifying questions.

Understanding of the principles and practices of public health surveillance, as well as evaluation techniques.

Knowledge and skills necessary to secure protected health information and prevent any unauthorized use or disclosure.

 

Qualifications:

Minimum:

Valid Florida driver’s license or able to get one within 30 days of hire.

At least two years of clients/patients interviewing experience to obtain relevant information.

At least two years of client/patient customer service experience.

At least two years of experience working in a healthcare setting and with communicable diseases.

 

Preferred:

Bachelors’ degree (from an accredited college, or university).

Bilingual in English, Spanish and/or Haitian Creole.

At least 6 months experience working with Tuberculosis (TB) patients.
