The Florida Department of Health (DOH) is responsible for the health and safety of all citizens and visitors to the state. As a public health agency, the Florida Department of Health must monitor the health status of Floridians, diagnose, and investigate health problems, and mobilize local communities to address health-related issues. The Florida Department of Health develops policies and plans that support health goals, enforce laws and regulations that protect the health of all residents and visitors, and link people to needed health care services. The Florida Department of Health also provides specialized assistance to pregnant women, infants, and children with special health care needs, regulates health care practitioners, and provides medical disability determinations.

 

Senior Community Health Nurse:

The Senior Community Health Nurse works primarily in the Tuberculosis Program. Working independently adhering to all established state and local protocols written/approved by the Florida Department of Health.

 

Your Specific Responsibilities:

  • Performs Tuberculosis screening of clients and contacts to Active Tuberculosis to include infants, children, and adults.
  • Provides Tuberculosis education and counseling to clients, contacts and the community.
  • Staffs clinic and serves tuberculosis patients
  • Performs hospital visits and home visits, as needed to admit the client into our services provide a monthly nursing assessment of infants, children and adults
  • Documents all encounters in the EMR, including recording service provided using the Health Management System database
  • Works closely with providers and Disease Intervention Specialists (DIS)
  • Performs laboratory functions as necessary to include phlebotomy
  • Reviews and interprets laboratory test results and contacts client with abnormal results
  • Coordinates client case management
  • Orders medications from Central Pharmacy and provides current medication plan and medications to DIS

  • Assumes responsibility for initiating care, delivering treatment to patients, and accurately documenting patient care in the EHR in timely manner.
  • Able to maintain/manage case records within time constraints. 
  • Able to bend, kneel, carry, lift, push or pull more than 15 pounds for community outreach events or other per program needs.
  • Basic computer skills proficiency level with Microsoft 365 applications (Outlook, Excel, Word, PowerPoint, and MS Teams a plus), and Internet or Internet/Intranet-based systems such as People First and Health Management System (HMS)
  • Ability to operate common office equipment including but not limited to: Copiers, scanners, fax machines, and telephone.

 

Qualifications:

Minimum:

  • Active Florida Registered Nurse License or a multistate license from another Compact State

Preferred:

  • Phlebotomy experience
  • One year or more nursing experience working with infants, children, and adults
  • One-year experience in case managing patients
  • Experience managing Tuberculosis (TB) patients
  • BSN or public health degree
  • Bilingual (fluent) in English and Spanish
  • Reliable transportation
  • Valid Florida driver’s license or able to get one within 30 days of hire
