Specific Responsibilities:

This is an Advanced Practice Registered Nurse Specialist (APRNS) at the West Palm Beach Health Center providing services in a health clinic to a low and high-risk urban population.  Services will be provided in Maternity, Family Planning, Sexual Health and GYN Clinics.  Work is performed under the supervision of the Health Center Administrator-SES.  This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions.  This position must travel to provide services at another satellite Center.

 

This position is designated as a sensitive position and required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocols and Procedures. The scope of practice will be within protocols established mutually between the Director and County Health Department guidelines and in conformance with the Specialized Certification.  Confidential Data Set(s): Clinical Information, Electronic Health Record (EHR), Electronic Lab (Emdeon), Electronic Prescribing (E-scribe), Surveillance Tools & Reporting System (STARS), Health Management System (HMS) TB & HIV Module. Health Management System (HMS) for the purpose of reviewing patient medical records, Electronic Health Record (EHR) for reviewing and documenting in the patient electronic record including e-scribe, e-lab and Surveillance Tools & Reporting System.  This position will view, enter and print data from the HMS Assessment Module based on established guidelines and release information.

 

Performs a complete physical appraisal of all patients appropriate to background and training.  Complies and evaluates medical histories and physical findings in the patient medical record.  Identifies and evaluates problems presented by patients.  Initiates treatment according to established protocols.  Monitors medications and alters medication according to variation patient’s response within established protocols, such as but not limited to pelvic exams, pap smears, venipuncture, skin tests, cultures and smears, Perform, manage and treat Prep patients.  Plans and conducts health education for patients and family including educational concepts in all phases of health care provided. Maintain follow up labs and refers as appropriate. Discusses any maternity abnormalities with physician and referrals to high risk care when necessary. Documents in real time in the Electronic Health Record.

 

This position will be amendable to precept students, training, attend conferences, meetings and activities associated with Maternity, Family Planning and STD.  Assist in the preparation of grants relevant to Family Planning at the discretion of Administration. Perform Quality Improvement Peer Reviews on a regular basis or Review records at the request of Leadership.

 

Participate in staff meetings, provider meetings, planning sessions, and in-service as assigned.  Assists with data collection to monitor program progress and participates in quality assurance functions.

 

Responsible for maintaining accurate, complete medical records required for completing medical and insurance reports and other necessary forms.

 

Dispense any medication covered by the APRNS’s protocol and prescribed to the patient directly. Dispensing shall be done in accordance with all State and Federal laws and shall be done following the policies and procedures of Florida Department of Health Palm Beach CHD including all associated record keeping requirements.  Accurately correct medication from stock, reconstitution of product if required, affixing a patient label to the bottle or entering the appropriate information on a pre-labeled container, providing the patient with a medication information sheet, having the patient sign the Offer to Counsel Signature Log, and handing the patient or their legal representative the medication. 

 

Participates in the collection of statistical data for program evaluation and audit purposes of the Quality Assurance Program.

Required Knowledge, Skills, and Abilities:

  • Intermediate to Advanced computer skills and knowledge of computer programs (Microsoft Word, E-mail, Internet and Excel)
  • Ability to establish and maintain effective working relationships, customer service skills, communicate verbally in a courteous and effective manner.

  • Knowledge of nursing principles, practices and techniques

  • Ability to initiate treatment plan according to established protocols

 

Qualifications:

  • Certification as an Advanced Registered Nurse Practitioner in accordance with Chapter 464, Florida Statutes, and in accordance with Rule 64B9, Florida Administrative Code, or certification as a Physician’s Assistant in accordance with Chapter 458, Florida Statutes.

  • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency.  Emergency duty required of the incumbent includes working in special needs shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
  • A Valid Driver’s License
