Specific Responsibilities:
This is a Community Health Nursing Consultant position assigned to the West Palm Beach Health Center. Responsible for the clinical areas and programs at the health Center including but not limited to HIV/AIDS, TB, STD, Refugee Health, Homeless Outreach Team screening, Nutrition, Laboratory, Pharmacy, Dental, Maternity, Family Planning, GYN and ADAP. Work is performed under the supervision of the Health Senior Physician This position supports the department and/or organization and works with others in an effort to accomplish the goals of the unit. Promotes the department as a positive, caring community organization through courteous behavior toward all clients and coworkers. This position works with patients of various backgrounds and cultures where language is often a barrier. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions. This position spends the majority of the time engaged in supervisory responsibilities. This includes communicating, motivating, training, and evaluating employees in addition to planning and directing employees’ work. The incumbent has the authority to hire, transfer, suspend, lay off, recall, promote, discharge, assign, reward, or discipline subordinate employees or effectively recommend such action. The incumbent will also be required to develop performance expectations, meet with staff regularly to discuss performance, complete performance reviews, maintain vacation and work schedules, ensuring proper coverage, and complete timesheets. Directs, in coordination with Center Administrative Team, the management operations within the Center. This position may travel to satellite Centers (Jupiter or Northeast) and other sites in Palm Beach County to provide supervision at the discretion of Administration.
This position is designated as a sensitive position and is required to maintain confidential information in accordance with Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocols and Procedures. This position will be managing staff and is responsible for sensitive information, financial responsibilities, etc. Strict adherence is required to the FDOH CHD Information Security Policy, Protocols and Procedures. Confidential Data Set(s): Clinical Data, Immunization Data, HIVe & TB, Human Resources Data, Social Security Data, Epidemilogy, HIV/AIDS Case Reporting and Surveillance, School Medical Record, WIC, Employee Health Record, Surveillance Toosl & Reporting System (STARS), Electronic Health Record (EHR), Health Management System (HMS), My Florida Marketplace (MFMP), Financial Budget Module, Pharmacy Forms System(PFS), FQHC Database, Q-Flow. This position will have access to the following system(s): HMS for the purpose of reviewing patient medical records, report portal, printing of schedules, reviewing patient medical visits and or services, EHR for reviewing patient medical records and updating and documenting in the record, Fla Shot for reviewing and printing of immunization records, HMS for HIVe Module to review and print progress notes and reports, HMS TB Module to review and print progress notes and reports, Internet, Financial Budge Module for reviewing and updating, My Florida Marketplace for creating and approving requisitions, Intranet In-Stock Inventory System (SIS) for creating and approving requisitions. This position will view, enter, update and release patient information for continuity of patient care and print data from the HMS.
Supports the unit, department and/organization and works with others in an effort to accomplish the goals of the unit, department, and /organization. Supervises and monitors staff’s work activities by evaluating service delivery in North County Clinics: WPB, Northeast and Jupiter. Assumes responsibility for staff in the absence of the supervisor as needed. Plans and makes supervisory site visits to assess and guide the performance of staff. Assigns staff working sites and scheduled hours. Serves as resource person for technical knowledge in the field of public health nursing or other nursing specialties. Provides direct supervision for Community Health Nursing Supervisors and their subordinate nursing and ancillary staff through observations, record review and individual conferences. Participates in the planning and orientation of new staff regarding to policies, programs and procedures. Advises and counsels staff about practicum planning for nursing students and other groups to public health and public health nursing in coordination with the division of nursing. Assumes lead responsibility for risk management and safety issues within the Center. Responsible for Employee Health, Incident Reporting and follow-up, Infection Control, including OSHA Compliance. Conducts regularly scheduled meetings with nursing staff, administration and management.
Provides administrative leadership and responsibility, in coordination with management team, for business practices within the Center as follows: Plans, monitors and evaluates patient flow activities, coordinates and monitors business activities relating to patient flow including scheduling, registration, data entry, and check in/check out billing. Coordinate authorizations for Medipass and other managed care organizations, as needed. Participates in Center financial management practices including budget process, billing, and contract/grant management, supply and inventory activities. In the absence of the Senior Physician, assumes full responsibility for the Center operations; this includes coverage for situations occurring after business hours. Plans, implements, and monitors quality improvements/quality assurance by conducting regular chart audits, peer review, handling complaints, planning for staff development, analyzing and reporting emerging problems or trends in the delivery of health services. Collects staff reports and review working documents checking for completeness and accuracy, making recommendations of such. Completes and submits assignments and professional reports without error at times stipulated by supervisor. Plans and assist with orientation of new staff. Provides new staff, for at least a month on clinical and technical support. Email communications are maintained daily and the accurate completion and timely submission of all administrative requirements, including Employee Activity Record (EAR’s), timesheets, sign in logs, reports, responses to ServeFL alerts, continuity of Operations Plan, call down list.
Assist and monitor the supervision and maintenance of the State Pharmacy program. Maintain a pharmacy document list indicating the physical location of all require documentation require during an audit or inspection including but not limited to, invoices, issue documents, transfer documents. This list shall include the physical location of the most recent complete pharmaceutical inventory conducted in the Community Health Development (CHD). Keep on file patient profile forms and hard copy prescriptions. Labeled accurately and signed by a provider when applicable all distributed medications in clinic. Re-order patient specific medications in addition to using the Pharmacy Forms System (PFS) for ordering and inventory control, an internal data base. Keep a perpetual inventory of pharmaceuticals up to date. As the Nursing Consultant and supervisor of the clinic supervisor ensure all nursing staff that has access to this pharmacy are trained appropriately and any error that occurs is reported immediately to the Center’s administration and to the State Pharmacy.
Monitor monthly employee performance in preparing for quarterly report and timely submission of quarterly Uniform Data System (UDS) reports. In the absence of the employee and or supervisor; ensures that the UDS report is submitted by quarterly established deadline date.
Monitors contract nurse performance. Ensure all deliverables of the contract are met, and all reporting requirements completed. Approve weekly timesheets, invoices, and deliverables for payables to the contractor. Monitor payables to not exceed the annual allowance of the contract.
Participates in organizational and community activities including committees, workgroups, task force boards, audits.
Participates in staff meetings, planning sessions, in-service as assigned. Assist with data collection to monitor program progress and participates in quality assurance functions.
Serve as the liaison with the Community Health Nursing Supervisor for the Refugee Health Screening Program with the State Refugee Program. Attends the county Refugee Task Force meetings.

Licensure/registration/certification requirements:
- This occupation requires licensure as a Registered Professional Nurse in accordance with Chapter 464, Florida Statutes and in accordance with Rule 64B9, Florida Administrative Code.
