Specific Responsibilities:

This is a Health Educator Consultant -SES position in the Community Health Promotion and Education Program. Responsible and accountable for the scope of work for the comprehensive chronic disease prevention and control program in Community Health Florida Department of Health Palm Beach County Health Department. Work is performed under the supervision of the Public Health Services Manager F-SES. This position will create or maintain stand-alone county tobacco-free partnerships, including local Students Working Against Tobacco (SWAT) chapters. Establishing policy and systems changes to eliminate secondhand smoke exposure. Establishing local policy and systems changes to promote cessation from tobacco use. This position serves as a liaison between the health department, Bureau Tobacco Free Florida, young and adult organizations, business, worksites, and other local agencies. Serves as a resource to assist individuals’ communities to reduce the burden of tobacco use and shape tobacco-free norms for tobacco to becomes less acceptable and accessible. This position may be required to transport youth and adult volunteers. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions. This position spends the majority of the time engaged in supervisory responsibilities. This includes communicating, motivating, training, and evaluating employees in addition to planning and directing employees’ work. The incumbent has the authority to hire, transfer, suspend, lay off, recall, promote, discharge, assign, reward, or discipline subordinate employees or effectively recommend such action. The incumbent will also be required to develop performance expectations, meet with staff regularly to discuss performance, complete performance reviews, maintain vacation and work schedules, ensuring proper coverage, and complete timesheets.

This position is designated as a sensitive position and is required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocols and Procedures. Confidential Data Set: Social Security and Outreach. Financial Information System (FIS) for the purpose of retrieving financial data and Health Management System (HMS) for purpose of Employee Activity Record (EARs) entries. This position will view, update confidential information.

Coordinates the implementation and evaluation of all tobacco prevention and control activities. Provide technical assistance and guidance; planning principles health promotion and practices required to meet the goals outlined in the workplan. Performs community needs assessments and surveys to identify strategies for Chronic Disease Prevention, adult use reduction and policy development to effectively control chronic diseases including tobacco use, access and exposure to secondhand smoke. Reviews and analyzes data for the purposes of program planning according to CDC Best Practices for Chronic Disease Program. Informs and educates individuals, stakeholders and communities on the benefits of evidenced-based policy, system and environmental changes using various communication methods.

Provides consultation and training to mobilizes, empower, and motivate stakeholders and communities. Works closely with business and worksites to promote and implement policy changes to establish smoke-free facilities and provide cessation services for their employees. Coordinates program activities with internal staff working in Chronic Disease Prevention, Maternal and Child Health, WIC, Dental, and other health department staff as needed. Provides support to the local tobacco-free community partnership by helping to organize meetings; developing media releases on key issues; assisting in recruiting partnership members; providing issue-specific trainings; conducting business; and planning and implementing community activities and events. Actively participates on local committees, boards, and coalitions to promote the mission of the agency and program.

Develops and submits the Quarterly and annual Tobacco prevention workplan. Ensure all documentation is accurate and complete for the reports. Submits report as required in the Automated Tobacco Activity Collection System (ATACS). Submits the Commitment Change Request as needed. Serves as the backup Contract Manager for any contractual services; prepares purchase order documentation and justification for vendors; and ensures compliance with State purchasing guidelines. Prepares reports, charts and graphs for informational and training purposes and community presentations. Implements the Plan-Do-Check-Act process for all strategies and interventions. Conducts process, impact and outcome evaluation methods to ensure program outcome measure and achieved. Oversees and supports all other programs in the Community Health Promotion and Education program in the absence of the Public Health Service Manager F-SES.
