The Agency for Persons with Disabilities works in partnership with local communities to support people with developmental disabilities in living, learning, and working in their communities. APD provides critical services and supports for customers with developmental disabilities to reach their full potential. The Agency serves people with spina bifida, autism, Down syndrome, children age 3-5 at high risk of developmental disabilities, cerebral palsy, Prader-Willi syndrome, Phelan-McDermid syndrome, and intellectual disabilities. DUTIES:

Employees of the Agency for Persons with Disabilities are required to demonstrate total commitment to outstanding customer service.  This includes, but not limited to, acting in a responsive, professional, courteous manner with the employees, customers and public we serve at all times.

 

This is a responsible professional position in which the employee is responsible for licensing and monitoring group homes that are licensed by the Agency for Persons with Disabilities (APD), and conducting wellness visits, as assigned. Monitoring visits are required monthly and licensing is required annually for all APD group licensed group homes.

 

Conducts monthly monitoring visits to assigned group homes, or more frequently, as directed, using the agency approved monitoring tool.  Participates in Wellness visits as assigned.  Identifies deficiencies and prepares Notices of Non-Compliance and follows up to ensure that required corrective actions are completed by the group home provider. Provides follow up by assessing whether the providers corrective actions address the non-compliance and reports lack of compliance to his/her supervisor. Provides follow up on Abuse Reports, Incident Reports, and any other reports as assigned.

 

Takes the lead for licensure renewals of APD licensed group homes  to assure 100% compliance of F.A.C. 65G-2, Chapter 393, F.S., Fire and Health Department Administrative Code.  Completes the consumer, personnel, and home licensing checklist, as well as verifying water temperatures and the licensed capacity.  Assists with staff compliance as required by reviewing all staff training records and background screening information.  Completes Initial Application process within 90 days of receipt of completed packet.  Schedules all re-licensing surveys by notifying the group home 90 days out of the scheduled date of the licensure expiration date.  The written notification includes attachments of all documents used in the licensure survey process. Prepares the required paperwork for signature by relevant staff in a timely manner. 

 

Provides technical assistance to each provider as required including, but not limited to, Group Home Workshops.  Coordinates or facilitates needed training for residential facility providers.  Attends and participates in monthly staff meetings, monthly meetings with Supervisor, Monitors, Medical Case Management Staff, Residential Planning Coordinators, and Behavior Analysts.

 

Updates iConnect system and all residential databases to reflect current licensure.  Maintain residential files on all APD facilities in Region/Field office.  Performs CEMP plan activities as required during disasters.

 

Performs other assigned duties as required. 

 

KNOWLEDGE, SKILLS AND ABILITIES

  • Knowledge of and ability to work directly with persons with developmental disabilities
  • Knowledge of and ability in monitoring and quality assurance
  • Knowledge of Microsoft Office Suite Software (Word, Excel, SharePoint, Access, PowerPoint)
  • Ability to travel for work purposes

BACKGROUND SCREENING REQUIREMENT

It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.
No applicant for a designated position will be employed or permitted to volunteer until the Level 2 screening results are received, reviewed, and approved by the Agency.
Level 2 background screening shall include, but not be limited to, fingerprinting for Statewide criminal and juvenile records checks through the Florida Department of Law Enforcement, and Federal criminal records checks through the Federal Bureau of Investigation, and may include local criminal records checks through local law enforcement agencies.
