Specific Responsibilities:

 

This is a Senior Licensed Practical Nurse position in the Immunization Program. This Senior Licensed Practical Nurse position requiring extensive knowledge of the prevention and control of vaccine preventable diseases and employee health.  This is countywide position requiring travel and the ability to work weekdays, evenings, and weekends on the Immunization Van outreaches, as well as assisting at the Health Centers as needed. Work is performed under the supervision of the Community Health Nursing Consultant. This position supports the unit, department, and/or organization and works with others in an effort to accomplish the goals of the unit, department, or organization. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions.

 

This position is designated as a sensitive position and is required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocols and Procedures. Confidential Data Sets: Clinical information, Epidemiology, HIV Aids Case Reporting & Surveillance, WIC, Employee Health Records and School medical records. This position has access to Health Management System (HMS), and FLSHOTS. This position uses Florida SHOTS to view, update, and print patient records and reports including the 680 Immunization Certification form and 681 Religious Exemption Form. This position uses Florida SHOTS to view and print patient records and reports, and to release the information according to HIPPA.

 

Provides immunization services using proper FDOH CHD policies, procedures and safety regulations. Provide immunization related education to all patients served, and employees as assigned. Assist with collection of data for audit purposes to meet agency reporting requirements for immunizations and all other clinical programs. Assist in orienting new nurses and works as a preceptor for new nurses in Immunizations and all other clinical programs as directed by supervisor.
 

•    Give appropriate vaccines/immunizations to children and adults, and enter vaccines given into HMS & Florida SHOTS data base.
•    Verifies patient identity before vaccines/immunizations are administered to ensure correct vaccines/immunizations are given as needed.
•    Ensure patients receives correct Vaccine Information Sheets (VIS) according to the vaccines/immunizations administered.
•    Complete 680 forms accurately
•    Assist with vaccine count and immunization reports as needed.
•    Assist with orienting new nurses to immunization program, and work as an immunization preceptor for new nurses.
•    Complete all other immunization duties as assigned by clinic supervisor.
•    Assist Immunization team with outreaches to include vaccine administration, education, documentation, and vaccine storage and handling.

 

Attends staff meetings and in-service programs as assigned. Participates in Quality Management as related to the immunization program.

 

Perform other duties as assigned.

Qualifications:

This occupation requires licensure as a Practical Nurse in accordance with Chapter 464, Florida Statutes or eligible to practice nursing in accordance with Florida Administrative Code 210-8.22 or 8.27.

 

Other Job-related requirements:

  • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency.  Emergency duty required of the incumbent includes working in special needs shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
  • Travel through Palm beach County is required.
  • Valid Driver’s License.
