Your Specific Responsibilities:
This position, an employee of the Nassau County Health Department (NCHD), works to protect, promote, and improve the health of all people in Florida through integrated state, county, and community efforts. This position reports to the Senior Public Health Services Manager, Director for the Division of Community Health Promotions. Operates a state/personal vehicle across the county and surrounding areas to participate in community events representing the NCHD.
- Serves as the Accreditation and Quality Improvement Liaison for DOH-Nassau. Leads in conducting, coordinating developing and implementing all aspects of Nassau CHDs foundational plans to maintain accreditation.
- Participates as a team member in the ongoing processes and improvements for maintaining accreditation across all PHAB domains and standards.
- Serves as an internal consultant regarding processes that can be improved upon, based on national standards. Routinely provides Foundational Plan updates to the Nassau CHD Division Directors and Performance Management Council.
- Serves as the Community Health Needs Assessment and Community Health Improvement Plan (CHIP) Liaison for DOH-Nassau. Leads and supports health improvement projects and initiatives identified in comprehensive CHA report.
- Provides project management and expertise to the CHIP steering committee with regular meetings, documentation of activities, and progress performance in accordance with the PHAB model. In coordination with the Community Health Program, Works in partnership with internal and external stakeholders to develop and implement community health improvement strategies based on in-depth knowledge of evidence-based practice, input from community partners, and local community assets.
- Present findings at community engagement events.
- Update all plan documents regularly and submit quarterly and annually per state timelines board.
- Serves as lead in developing, implementing, and organizing all aspects of the agency’s Workforce Development plan and PMQI plan.
- Empower all internal partners to participate, communicate and document goals, objectives, and activities.
- Participates in process improvement and problem-solving opportunities to enhance department efficiency and effectiveness.
- Routinely provides Strategic, Workforce Development and Quality Improvement Plan updates to the Nassau CHD Division Directors, Performance Management Council and during All Staff Meetings.
- Present findings and identify training needs for internal improvement across all five CHD divisions.
- Update all plan documents regularly and submit quarterly and annually per state timelines board.
- Collaborates with community partners to achieve mission and vision objectives of DOH-Nassau Health Officer.
- Maintains scorecard of strategy activities and updates internal SharePoint website monthly.
- Performs all other duties as assigned.
Required Knowledge, Skills, and Abilities:
- Ability to use Microsoft applications to analyze data and create reports.
- Knowledgeable of Health Department policies and procedures.
- Knowledge of the Community Health improvement framework and processes.
- Knowledge of Community Health Assessment and Prioritization processes.
- Ability to lead, develop, and support group projects.
- Skills at identifying leads from external agencies and engaging community partners to achieve a common goal.
Qualifications:
Minimum-
- Proficient experience with Microsoft Office 365 applications.
- Experience analyzing data and preparing reports for internal and external stakeholders.
- Experience with public speaking and presentation planning.
- Bachelor’s degree in health Promotion, Health Education, or Public Health or other health-related degree and/or equivalent.
- Professional experience in Data Analytics, Planning and providing health education services.
- WORKING HOURS: Daily from 8:00 am to 5:00 pm Total hours in workweek 40.
- Emergency duty required of the incumbent includes working in special needs shelter or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
- Incumbent must possess a valid Florida Driver License and have a reliable motor vehicle to carry out these duties.
Preferred-
- Experience providing technical assistance, consultative services and direction for development, implementation, evaluation, and expansion of programs of service in public agencies and local governments.
- Minimum -Bachelor’s degree from an accredited college or university.
- Knowledge of principles and processes for providing customer and personal services including needs assessment techniques, plan development, quality service standards, alternative delivery systems, and customer satisfaction evaluation techniques.
