Your Specific Responsibilities:

This position, an employee of the Nassau County Health Department (NCHD), works to protect, promote, and improve the health of all people in Florida through integrated state, county, and community efforts. This professional position directly reports to the EXECUTIVE COMM HEALTH NURSING DIR – SES, Disease Control Program, and DOH Nassau. This position is the lead worker for these duties and is expected to perform independently, without routine direct supervision, to carry out duties and responsibilities. As lead worker routine duties require priority setting, oversight, delegation and expert consultation for staff assigned to the program. As lead, this position also participates in monthly budget monitoring reports to the Ryan White Part B grantee. This position is required to provide program related quality improvement (QI), data analysis and HIV case management for clients served by the Ryan White Part B grant and/or the DOH state AIDs Drug Assistance (ADAP) programs. This position also provides community health planning and surveillance support as directed by the CHD Director and the Disease Control supervisor.

Services include supportive counseling, referral to needed HIV health-related and social service resources, with a focus on HIV care linkage, activities to assure retention in care, and medication adherence. HIV care linkage services are provided in the office, clinics and through home visits. Data are entered into HMS, CAREWARE, and ADAP (PROVIDE) or other DOH required recording systems. All services and data are documented accurately and completely using the required forms and formats, within one business day after service is provided. Maintains an appropriate case management leveling system for cases, to include management of HIV related client level care plans.  Conducts program related QI projects, including publishing findings. Provide periodic reports, including descriptive data analysis on client outcomes and program benchmarks, as requested by supervisor. This position works collaboratively with the ADAP program support registered nurse. This position is responsible for assuring medication is processed through ADAP drug assistance program software (PROVIDE) and medications are distributed to patients in the ADAP program. Completes EARS weekly in a timely manner to meet or exceed state goal of 95%.  Completes all required trainings as needed for professional development.  Attends meetings, seminars and training to program updates with supervisor's approval. Responsible for attendance at regional peer HIV case manager meetings.  May operate a state vehicle as part of duties and responsibilities in and around the Nassau County area. 

Other health department support duties as assigned including for surveillance support using DOH ESSENCE and MERLIN software. Will create ESSENCE queries and reports to monitor adverse health trends for non-communicable diseases in Nassau County, as requested by the CHD Director or supervisor. This includes adverse trends related to behavioral health, injury-related morbidity or mortality. This position will be able to provide back up to DOH Nassau Epidemiologist for MERLIN and ESSENCE (e.g., can review standing pre-set queries and notify alerts to Disease Control Manager or designee).

Required Knowledge, Skills, and Abilities:

  • Knowledge of practices in counseling and social work
  • knowledge of professional ethics in relation to education and supportive counseling, motivational interviewing techniques
  • Ability to develop plans of case for cases
  • Ability to organize work assignments and work independently and in teams
  • Ability to actively listen and communicate effectively, to establish and maintain effective working relationships with others.
  • Ability to use a computer for data entry. 
  • Knowledge of Microsoft Office Suite. 
  • Skill in oral and written communication. 
  • Ability to perform home visitations using county vehicle/personal vehicle. 
  • Ability to lift to 20 pounds

 

Qualifications:

Minimum-

  • Service oriented individual.
  • Knowledge of principles and processes for providing customer and personal services.
  • Knowledge of health promotions and disease preventions.
  • Knowledge of the transmission, prevention and risk reduction of STDs, HIV/AIDS, Hepatitis.
  • Skill in motivational interviewing and counseling.
  • Knowledge of contact investigation, tracing and notification of communicable diseases.

Preferred-

  • Bachelor’s degree in social work, social sciences (psychology, human services, sociology) or a health-related field from an accredited university.
  • At least 1-year professional experience providing social work, nursing or counseling services.
  • Bilingual – English and Spanish
