Your Specific Responsibilities:
This position, an employee of the Nassau County Health Department (NCHD), works to protect, promote, and improve the health of all people in Florida through integrated state, county, and community efforts. Employee will be operating a state vehicle to service clients within the Nassau County area. This position reports directly to the Executive Community Health Nursing Director-SES.
Clinical Nursing & Case Management:
- Provide direct services to clients in the following programs: Immunization, Tuberculosis, Family Planning, Primary Medical Care, and STD.
- Provide independent nursing services under written Nurse Protocols.
- Assess and evaluate and provide care within scope of practice per FL Nurse Practice Act.
- Utilize nursing skills of interview, observation, and assessment to determine the medical needs of the client and determine specific risk factors for client.
- Triage client to meet their needs in timely manner.
- Lead Nurse for Mobile Health Van and Medical Clinic.
- Perform laboratory testing, including venipuncture.
- Prepare specimens for transport to outside laboratory.
- Record client data per electronic health record and Florida Shots Registry
- Provide case management to established client and make appropriate referrals.
- Monitor client follow-up to assure it is completed or document why it is not completed.
- Review and report labs per protocol.
- Utilize client EMR to document follow-up per guidelines.
- Provide clinical support to provider and team as requested.
- Record client data per electronic health record and Florida Shots Registry.
- Maintain confidentiality of client interactions and services according to HIPPA privacy policies
- Work with HIV or Ryan White program as needed.
Clinical Maintenance:
- Assist in maintenance of clinic supplies.
- Conduct monthly inventory of pharmacy meds and immunizations per guidelines.
- Assure forms and handouts are kept up to date for assigned clinic.
- Ensure a safe and healthy environment by adhering to OSHA guidelines.
Clinical and Program Oversight:
- Provide program oversight to assure all requirements are met for programs assigned.
- Submit all reports due for programs assigned.
- Attend program calls and meetings for assigned programs.
- Attend staff and other internal and external meetings as assigned.
- Attend training for personal and professional growth.
- Participate in quality improvement activities, including chart reviews.
- Complete chart reviews as required.
- Participate in QI projects as assigned.
- Responsible for Nassau DOH on call duties per guidelines on a rotating basis.
- Performs all other duties as assigned.
Required Knowledge, Skills, and Abilities:
- Ability to assess clients and make sound nursing judgments independently.
- Knowledge of nursing and public health principles, practices, and techniques.
- Ability to plan, organize and coordinate work assignments in a prioritized fashion.
- Ability to maintain accurate and appropriate medical documentation in EHR system.
- Ability to safely administer medications and immunizations within scope of practice.

Qualifications:
Minimum-
- Licensure as a Registered Professional Nurse in accordance with Chapter 464, Florida Statutes and in accordance with Rule 64B9, Florida Administrative Code
Preferred-
- Licensure as a Registered Professional Nurse.
- Bachelor's degree in Nursing
- 2 years’ experience in a clinic or community setting.
- Knowledge of the information and techniques needed to diagnose and treat injuries, diseases, and deformities which to include symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures.
